SOCIAL WORK at soul pham support services

Specialist, ethical, and person-centred NDIS support

At Soul Pham Support Services, our Social Worker, Wende is a university-qualified allied health professional who supports people with disabilities to strengthen their wellbeing, relationships, and participation in everyday life.  Wende works alongside participants, families, carers, and other professionals to navigate complex systems, reduce barriers, and support meaningful, sustainable change.

What does a Social Worker do?

Social Workers support people to maintain or improve social functioning, the skills and supports needed to:

  • Build and maintain relationships
  • Navigate family, community, and service systems
  • Manage life transitions and change
  • Strengthen emotional wellbeing and resilience 
  • Participate in education, employment, and community life

Social workers are particularly skilled in working with complex presentations, including psychosocial disability, trauma, family violence, parenting stress, and systemic disadvantage.


Social Workers are trained to complete a wide range of NDIS-relevant assessments, including:

  • Psychosocial assessments
  • Functional capacity assessments
  • Risk and safety assessments
  • Needs and support assessments
  • Social functioning assessments
  • Independent living and environmental assessments
  • Carer and family assessments

These assessments support planning, reviews, and evidence-based recommendations aligned with NDIS requirements.

How Social Workers support NDIS participants

Our Social Workers provide Improved Daily Living Therapeutic Supports, tailored to each participant’s goals and circumstances.  Supports may include:

  • Therapeutic counselling and capacity-building 
  • Practical problem-solving and goal setting 
  • Support through life transitions (housing, family change, school, employment) 
  • Advocacy and system navigation 
  • Strengthening informal and formal support networks 
  • Collaboration with multidisciplinary teams 

We also work with families and carers to build understanding, skills, and confidence, helping to reduce pressure within the support system around the participant.


Qualifications and professional accountability

Wende has a Master of Social Work and has completed 1,000+ hours of supervised placement. She is a member of the Australian Association of Social Workers (AASW).  Wende adheres to the AASW Code of Ethics and Practice Standards and participates in ongoing professional development and clinical supervision.
